The filing seeks an overall 4.1% rate increase, with rates requested to take effect January 1, 2026. The submission is structured as a single case large group negotiated rate filing with no form deviations and applies to one vision plan offered through a standard large group master eye care contract.

According to the filing, the group consists of 75 eligible employees, with 68 enrolled, meeting Washington’s large group eligibility threshold. Updated monthly employee rates reflect the proposed increase across coverage tiers, including employee only, spouse, child, and family options.

Ameritas notes that supporting actuarial detail tied to group specific experience was submitted separately as non public material, citing confidentiality around its rating methodology.
